• Complete new patient intake paperwork • Provide autism diagnostic evaluation report (updated every 3-5 years, dependent on your insurance) • Provide ABA referral from a physician or psychologist (within one year) • Provide recent medical summary notes (within one year) • Provide insurance information • Provide IEP (if applicable) • Review and sign policies and procedures in Client Handbook • Confirmation of schedule and participation in weekly caregiver support

• Complete a home safety check • Complete Vineland Adaptive Behavior Scales questionnaire • Attend initial intake meeting with BCBA or Autism Specialist (AS) • Standardized assessments: ESDM, ABLLS-R, PEAK • Direct observations across home, school, daycare, and community • Create an individualized treatment plan
